Frailty Team

The Frailty team sits within the NHS Gloucestershire Integrated Care Board. The team have developed a Frailty Training and Education Framework and an online Frailty & Dementia Practitioner Toolkit – links to these free resources can be found within the Useful Links section.

The Frailty and Dementia Toolkit is a resource for all practitioners working with people living with frailty and/or dementia, whether they are from a clinical or non-clinical background. It is intended to bring together resources, tools, advice and guidance to share best practice and to support colleagues in their day-to-day work.
